Verq Check Privacy Policy

How Verq Check handles data. The short version: your patients' identities never reach our servers.

Effective: October 1, 2026 · Verq, Inc., 12620 Beach Blvd, Ste. #3-279, Jacksonville, FL 32246

Scope of this policy

This policy describes how Verq, Inc. ("Verq," "we," "us") handles information in connection with Verq Check — the pre-signature note-review browser extension for WebPT and other supported electronic medical records (EMRs), and its related iOS/iPadOS application (together, the "Service"). It covers the Service only. Verq's other products and our corporate website are covered by the separate Verq Health Privacy Policy. Where you install Verq Check through the Apple App Store, Apple's own App Store privacy disclosures also apply, and the practices described here are consistent with the privacy information shown on our App Store listing.

Single purpose. Verq Check has one purpose: pre-signature review of the outpatient therapy (PT, OT, and SLP) note you have open in your electronic medical record (EMR), to surface potential Medicare documentation and billing issues before you sign. Everything below describes the data handling in service of that single purpose and nothing else.

The short version

  • no accountno name, email, or password is collected to use Verq Check
  • no phi to our serversnotes are de-identified in your browser before anything is sent
  • not sold, not for ad trackingyour data is never sold and never used to track you across apps or sites
  • not used to train modelsyour documentation is never used to train AI models
  • you can reset and deletereset your device ID in the extension, or ask us to delete your organization's data

Information we collect

These are the only categories the Service handles. None is linked to your identity, and none is used to track you across other apps or websites.

CategoryWhat it isLinked to you?Used for tracking?
Health dataDe-identified clinical note content: ICD/CPT codes and de-identified SOAP narrative text, sent for reviewNoNo
Device identifierA random installation ID generated on first run, used to tie an installation to its organization licenseNoNo
Product interactionAnonymous counts of findings rendered, and per-license daily usage countsNoNo
Other dataBrowser, extension version, and platform; a truncated user agent and source IP recorded when a license or this agreement is acceptedNoNo

To review a note, you grant Verq Check access to your EMR's documentation site. It then reads the clinical note on the page you have open — and only that page, on the EMR sites you authorize. Verq Check supports WebPT today and is expanding to additional EMRs; it requests access only to supported EMR documentation sites and to Verq's own analysis service, and does not read other websites, your login credentials, or your browsing activity.

What we do not collect

We do not collect your name, email address, or password; we do not receive protected health information on our servers; and we do not collect your precise location, your contacts, your browsing history, financial or payment-card data within the Service, or crash/diagnostic telemetry (the Service includes no crash-reporting or advertising SDK).

How we use information

We use the information above only to provide and operate the Service: to review the open note and return findings; to validate and enforce your organization's license; to keep the Service compatible, secure, and working; and to answer your support requests. We do not use your information for behavioral advertising, and we do not sell, rent, or license it.

Chrome Web Store Limited Use

Verq Check's use of information received through the browser extension, and from any Google APIs, adheres to the Chrome Web Store User Data Policy, including the Limited Use requirements. Information accessed through the extension is used only to provide and improve the single purpose described above. It is never sold; never used for advertising, or for any credit, lending, or eligibility decision; never transferred to data brokers or information resellers; and never used or transferred for any purpose unrelated to that single purpose, except as necessary to operate or improve the Service, for security, or to comply with applicable law.

Apple App Store and tracking

The Verq Check app for iPad is distributed through the Apple App Store, and the practices described here apply to it as well as to the browser extension. Verq Check does not track you across other companies' apps or websites: it uses no advertising identifier (Apple's IDFA), requests no App Tracking Transparency permission, and sets no advertising cookies or SDKs. No account, name, or password is required to use Verq Check. To delete the limited data tied to your organization's license, use the in-product reset or email privacy@verqhealth.com. The privacy information shown on our App Store listing reflects these same practices.

De-identification

Before any clinical content leaves your browser, Verq Check removes direct identifiers — including name, date of birth, Social Security number, telephone number, email address, street address, medical record number, member or policy number, and facility name — and transmits only a de-identified subset (diagnosis and procedure codes and de-identified narrative text) for analysis. De-identification is performed in the browser on a commercially reasonable, best-efforts basis. How that works

AI processing and sub-processors

Verq Check analyzes the de-identified note using automated and artificial-intelligence techniques, including a third-party AI provider, OpenAI, with which we maintain a Business Associate Agreement and data processing terms. OpenAI receives only de-identified content, does not receive PHI, and does not use your content to train its models.

We share information only with the service providers that help us operate the Service — our AI provider and our hosting/infrastructure provider — each under a written agreement that requires them to protect the information at a level consistent with this policy and to use it only to provide services to us. We may also disclose information if required by law or legal process, to protect rights and safety, or to a successor in a merger, acquisition, or sale of assets. The current list is on our sub-processors page. We provide equal or greater protection obligations on any third party with whom data is shared.

When subscription billing is enabled, payments are processed by Stripe, Inc. Stripe receives your billing information only; it never receives any clinical content or note data.

Retention and deletion

Findings are shown in the panel in your browser and are not kept as an admin-visible record. De-identified request data and license and usage records are retained only as long as needed to operate and account for the Service, and are then deleted or further de-identified. You can reset your installation's device identifier at any time from the extension's own control, which also clears the license from that installation. To request deletion of data associated with your organization's license, email privacy@verqhealth.com; we will respond within 30 days.

Your rights and choices

Because Verq Check acts largely as a service provider to your organization, requests from individual clinicians or members are generally directed to the organization that licenses the Service. For information we hold directly, you may request access, correction, deletion, or portability, or opt out of non-essential communications, by emailing privacy@verqhealth.com. California residents have additional rights under the CCPA/CPRA, including the right to know, delete, and opt out of sale (we do not sell personal information). We will not discriminate against you for exercising these rights.

HIPAA and protected health information

When Verq handles protected health information on behalf of a covered entity, Verq acts as a HIPAA Business Associate under a Business Associate Agreement (BAA), which is included at signup. By design, de-identified data is all that reaches our servers; where a reportable security incident involving PHI occurs, we notify the covered entity within the timeframe set in the BAA, and in any event no later than 72 hours after confirmed discovery.

Security and data location

We protect information with encryption in transit (TLS 1.2+) and at rest, least-privilege access controls, and vendor agreements that forbid using your data to train models or for any purpose other than providing the Service. Information is stored and processed in the United States. No security measure is perfect, and we cannot guarantee absolute security.

Children's privacy

Verq Check is a professional tool for healthcare providers and is not directed to children. We do not knowingly collect personal information from anyone under 18. If you believe a minor has provided us information, contact privacy@verqhealth.com and we will delete it.

Website analytics

Our marketing website (verq.ai) may use privacy-respecting, first-party analytics such as Google Analytics 4 to understand site usage. We do not run advertising trackers on the site, and the Verq Check extension and app set no advertising cookies. You can limit analytics through your browser settings or Google's opt-out add-on.

Changes to this policy

We may update this policy as the Service or the law changes. For material changes we will update the effective date above and provide notice at least 30 days before the change takes effect, within the Service or at this page.
